Depending on what your request for protection pertains to you will need to have one of the following forms completed:

The WH380E is used if the absences are due to your own serious health condition.

The WH380F is used if the absences are necessary to care for a qualifying family member.

The WH384 is used to certify the need for leave due to a qualifying exigency.

The WH385 is used to certify the need to provide care for an ill or injured servicemember.

These are new forms effective January 2009, all previous forms will not provide adequate certification so I strongly advise that you not use any other forms.
